Amateur MMA Combatant License
To apply for an Amateur Combatant License in Arizona, the following is required:
- License Application Form submitted via DocuSign
- $10 Application Fee paid online via Payment Portal
- Physical Examination (Administered by an MD or DO): Form
- Dilated Eye Examination (Administered by an Optometrist or Ophthalmologist): Form
- Bloodwork (HIV, Hepatitis C Antibody, and Hepatitis B Surface Antigen)
*For Combatants 36 years and older, a Stress Test accompanied by an EKG and Interpretation Letter from an MD or DO providing clearance for Combat Sports.

Professional Combatant License
*If you are an amateur wishing to turn pro, please ensure you meet the minimum requirements found here.
To apply for a Professional Combatant License in Arizona, the following is required:
- License Application Form submitted via DocuSign
- $25 Application Fee paid online via Payment Portal
- Physical Examination (Administered by an MD or DO): Form
- Dilated Eye Examination (Administered by an Optometrist or Ophthalmologist): Form
- Bloodwork (HIV, HEP C Antibody, and HEP B Surface Antigen)
Cornerman License
To apply for a Cornerman License (i.e. Trainer, Second, Cutman) in Arizona, please complete the steps below:

- $25 Application Fee paid online via Payment Portal
Matchmaker License
To apply for a Matchmaker License in Arizona, please complete the steps below:
- License Application Form submitted via DocuSign
- Submit Fingerprints to the Arizona Department of Gaming *You may complete Fingerprinting at your local Police Department and mail two copies to our office at 100 N. 15th Ave, Suite 202, Phoenix, AZ 85007.
- $125 Application Fee + $22 Fingerprint Processing Fee paid online via Payment Portal
Announcer License
