Instructions for Applying for an Event Wagering Operator License
- If you are applying for an Event Wagering Operator, Designee, or Management Services Provider license during the current Event Wagering Operator Application Period, in addition to the business application, you must submit the Supplemental Allocation Application and individual disclosure forms for Directors and Officers, i.e. President, Vice President, CEO, COO, CFO, Controller, etc. Your application will not be considered complete unless the business application, the supplemental allocation application, all individual disclosures, payment and all other required documentation is received.
- During our initial review, if any other individual disclosures are needed, we will notify the main contact listed on the business application.

- Please follow the link on our website under the EWFS dropdown to make payment online (link is external).
- ADG will review the applications and supporting documents and make a licensing determination.
