What is Self-Exclusion?
- Self-exclusion is a tool that allows a person to request to be banned from either all Arizona Tribal Gaming Facilities or Event Wagering & Fantasy Sports (EWFS) Contests within the State of Arizona. If preferred, individuals may opt to self-exclude from both tribal casinos and EWFS contests. Both exclusions provide that a person on the self-exclusion list will be prohibited from collecting any winnings or recovering any losses. This self-ban includes the use of any of the services or privileges of the facility, such as restaurants, concerts, conventions, and hotels. The time period options available to Arizonans are either a one-year, five-year, or ten-year exclusion. Please note: the self-exclusion is irrevocable and cannot be altered or rescinded for any reason during the time period selected on the form.
- The Arizona Department of Gaming offers two types of self-exclusions: Casino self-exclusion - where you can choose to exclude yourself from gambling at all Arizona casinos, as well as EWFS self-exclusion - where you can choose to voluntarily exclude yourself from placing wagers or purchasing fantasy sports contest entries at any licensed facility or on any internet/mobile platform.
- These exclusions are separate; if you exclude yourself from Arizona casinos, you are not excluded from EWFS and vice versa. However, you can choose to complete both self-exclusions if preferred.
How Do I Exclude Myself From Arizona Casinos or Event Wagering & Fantasy Sports Contests?
- There are several ways you can go about excluding yourself. The forms (available in a fillable PDF) can be found in three separate sections of this website: on the DPG home page, the the navigation bar, and the Resources page. The Event Wagering & Fantasy Sports Contests specific self-exclusion form can also be found on the EWFS tab.
- You can download the appropriate self-exclusion form on this webpage, fill it out in its entirety, have it notarized, and then mail it to the Arizona Department of Gaming along with a current photo of yourself in color ink. Please note: the self-exclusion will not be processed without proper notarization and a current color photo.
- The Department is willing and able to provide notary services to those who request a notary. Photos may be submitted electronically via email; however, the original, notarized self-exclusion form must be mailed or delivered directly to the Arizona Department of Gaming’s main office: 100 N. 15th Ave., Suite 202, Phoenix, Arizona 85007. If submitted online, color pictures can be emailed and/or attached to the form.
- In-person self-exclusions are currently available Monday through Friday from 8:30 AM until 4:00 PM. Please note that you can choose to call and schedule an appointment, and we will assist you by walking you through the form and notarizing the self-exclusion for you, as well as taking your photo, all in the same day. Please do not let this time frame stop you from excluding yourself or receiving the help you may need, as we can be flexible and adjust if necessary.
- The Arizona Department of Gaming has recently added a remote notary option as another option to notarize the self-exclusion form. Please review the FAQ list to see what technology requirements are required and the acceptable forms of identification for the remote notary option.

Frequently Asked Questions
Can I put my spouse/child/parent/friend on the list?
- No. This is a self-exclusion program and only the problem gambler can elect to place his or her name on the self-exclusion list.
How do I remove my name from the list?
- Your name is removed only when the self-exclusion duration ends. There is no process to alter or rescind the self-exclusion. It is irrevocable and remains in effect for the entire time period elected. Your name will remain on the self-exclusion list until you complete the entire exclusion period, regardless of any change in personal circumstances.
What facilities will I be excluded from?
- The state-wide self-exclusion includes all Class III Tribal Gaming Facilities in Arizona and includes the use of services of any Tribal Gaming Facility, such as restaurants, concerts, hotels, and conferences on the casino property.
Will I be excluded only from Arizona casinos?
- Harrah’s/Caesars Entertainment has a policy that any person excluded from one of their casinos is excluded from all of their casinos, worldwide. Since Ak-Chin is a Harrah’s property, this means you will not be able to enter any casinos owned and/or operated by Harrah's or Caesars Entertainment anywhere. Once you exclude, Harrah's will send you a letter with a listing of their current properties.
How long does it take for the self-exclusion to go into effect?
- When the properly executed self-exclusion form and picture are received in this office, it is usually processed the same day. The Tribes have direct access to the database system. You are not notified when the exclusion is received in this office or when it goes into effect. It is recommended that you consider yourself banned from all Arizona casinos as soon as the form has been mailed or call the office to confirm.
What if I violate my exclusion?
- Once a self-excluded patron is identified in the gaming facility they will receive a consequence at the Tribal Gaming Office’s discretion, which may include the issuance of a citation and/or arrest and prosecution by Tribal or other authorities. A self-excluded person on the premises will be considered trespassing.
Will you notify me when the self-exclusion is over?
- No expiration notices are sent to self-exclusion participants. Tribal Gaming Offices and casino personnel maintain a list of active self-exclusions. Can I extend my self-exclusion for another year?
- Once your self-exclusion time period has expired, you may renew your self-exclusion for another one, five years, or ten years by sending in a new notarized self-exclusion form and providing a current picture.
Can I self-exclude in person?

Remote Notary FAQ's & Steps
- The remote online notary option is not for everyone. Users will need to be tech savvy. The remote notary option can only be used for individuals with a valid US State Driver's License or State ID. We are unable to use passports, green cards, visas, or other alternative forms of ID for the remote notary option. If this is your only form of ID you will need to find other means to have the form notarized.
Signer Requirements
- You will be required to have a computer/tablet with a camera AND a cell phone with camera.
- You will be required to answer a series of questions (pulled from a credit report) in order to verify your identity. You will also need to have your US State Driver's License or ID card at the time of the notary session as you will be prompted during the session to take a photo of the card front and back.
Steps to Request an online notary session for completing the self-exclusion paperwork.
- You will need to submit your completed self-exclusion paperwork WITHOUT your signature or date and a clear photo of yourself (head and shoulders) to [email protected]
- Once your completed paperwork is received, a staff member will contact you via email, to schedule the appointment within 1-3 business days. Same day remote notary sessions are not available.
- When your scheduled session is ready to begin you will receive an email with a link to join the live online session and you will need to follow the prompts and links within the emails received. Please plan up to 15-20 minutes to complete the process. We are unable to walk an individual through the process. If we are unable to complete the on-line notary session you will need to get the form notarized in-person.
Please note that because these sessions are live online, if you have not initiated the session within 5 minutes of your scheduled time, you will need to contact the staff member to discuss rescheduling the appointment.
