You need to provide your medicals (eye, physical, and bloodwork), the license application, documentation of citizenship or alien status, and pay the licensing fee ($10 for amateur MMA and $25 for Professional Boxers and MMA)

1. Submit Ticket Manifest and Audit Form to commission via [email protected]

2. Await confirmation from commission

3. Once reviewed and approved, submit payment via adgpay.az.gov/content/boxing-mma
